Assessment Forms
Charitylog has a module which allows you to build assessment forms. The most well known of these are the carer self assessment forms being developed around the country.
You are able to create the forms, along with scoring methods to produce either a financial entitlement, or a set of numerical scores, as seen in the Adult Carers Quality of Life Questionnaire (AC-QoL).
It is possible to have subsets of scores, for example a sum of the positive and negative item scores as well as the overall score.
Forms can have tick boxes, or other types of data, such as numbers, dates and text.
