The Wellcheck Scheme

Charitylog ScreenshotThe Wellcheck Scheme is similar to other schemes for older people where clients are visited, and their needs assessed and then referrals made to other internal or external services. In Worcestershire this scheme is run in conjunction with 7 local Age Concern offices, who are required to provide standardised and timely data returns.

Charitylog was already in use in some Age Concern offices in Herefordshire, Worcestershire and North Birmingham and was seen as a method through which data could be collected. The scheme was planned, developed and implemented through early 2006 and went live on Charitylog in April.

The Wellcheck Scheme consists of four main stages:

  • Initial referral of the client to the Wellcheck Scheme
  • Make a 'First Contact' to determine whether they are suitable for the service
  • Visit the client and carry out an Assessment which might also generate referrals into other Age Concern provided services as well as external services
  • Follow up the Assessment with a Review, leading to a series of further reviews or re-assessments

Programming was done to allow Charitylog to provide a structured path through these stages, as well as ensuring standardisation of data being presented to WSS.

A series of special reports, exported as Excel spreadsheets, was also created providing the data required by WSS to audit the scheme.

Since the various stages are required to occur at specific times, the normal Charitylog Action Lists were updated each time an event occurred to ensure that the next stage was not overlooked. Obviously all this information was also stored in the client's history, alongside any other activity.
